Ultrasound-guided injection of BTX-A for hypersalivation is effective, and side effects are rare, but they have yet to be fully described. Introduction Botulinum toxin A (BTX-A) inhibits the release of acetylcholine at cholinergic neuromuscular junctions and has been used to manage spasticity in patients with cerebral palsy.

They include lesions that arise uniquely in this location (eg, ranula, submandibular duct obstruction) as well as various malignancies, inflammatory processes, and vascular abnormalities that may also occur elsewhere in the head and neck. Sialadenitis is an infection of the salivary glands. It is usually caused by a virus or bacteria. The parotid (in front of the ear) and submandibular (under the chin) glands are most commonly affected. Sialadenitis may be associated with pain, tenderness, redness, and gradual, localized swelling of the affected area. The submandibular gland is the second largest (approximate weight, 10 g) of the major salivary glands (the parotid gland is the largest). Anatomically, it is situated in the submandibular triangle of the neck.

METHODS The parotid and submandibular glands of 10 patients were injected with BTX-A using ultrasound guidance. Before injection, the baseline rate of salivation was assessed using a submandibular spaces, and can be broadly classified into congenital, infectious, vascular and neoplastic aetiologies (Table 1). CT and MRI are the modalities of choice for imaging this region; CT is readily available, and MRI provides superior soft tissue resolution. Ultrasonographic examination of submandibular swelling showed well-defined cystic lesion with thick wall measuring 1.13 × 1.27 cm (Figure 2).
